To Apply for a New Adult Passport please follow the simple instructions below and contact us so we can begin the process in our system and review all the procedures to insure superior customer service.

Please prepare the following documents: 

Note:The following documents need to be 'Executed & Sealed' at the nearest Post Office ($25.00) or Clerk of Court, then we are able to expedite the processing for you.

1 - Passport Application DS-11 – Must to be completed online and downloaded from the Government Website. Please print application and fill out by hand. Do Not Sign. This must be signed at the Post Office when Executed & Sealed.

2 - Check or Money Order for $170.00 – made out to: The U.S. Dept. of State

3 - Original Birth Certificate – This will be returned with your Passport. 

4 - Signed Letter of Authorization – Must sign and have on top of paperwork. This will be sent to by a Customer Service agent.

5 - Two Passport Photos – Taken within the last 3 months.

6 - Copy of ID or Driver’s License – Include as second page.

7 - Itinerary – A copy of your Flight Plan. If you have not scheduled your travel plans yet please let us know.

Once these documents have been 'Executed & Sealed' please send to our office for expedited processing.
